docs: add setup templates and checklists for recurring tasks
Create SETUP_TEMPLATES.md with copy-paste templates for:
- New SvelteKit web apps (hooks.server.ts, getAuthUrl pattern, Dockerfile)
- New NestJS backends (schema, health, CORS)
